The pharmacy team form part of a rapidly expanding, progressive multidisciplinary PCN team currently consisting of five pharmacists and two pharmacy technicians supporting our six medical Practice.
We work collaboratively with other healthcare professionals across the network to ensure appropriate and safe medicines use. We do this by facilitating medication supply, delivering structured medication reviews, supporting clinical reviews of our 70,000 patient population.
This work supports GP workload and the burden on practices who deal with large volumes of medication queries for patients and care home residents.
