PCN Physician Associate Team
As of April 2021, we have appointed 8 Physician Associates to join our PCN team, 4 of whom have dedicated time to care for our care home population.
Each care home who is under the care of the PCN has been allocated a physician associate whose role includes overseeing the weekly ward rounds, initiating, and reviewing personalised care plans, completing chronic reviews, implementing advanced care planning and much more.
Physician Associates support doctors in the diagnosis and management of patients. They are trained to perform several roles including taking medical histories, performing examinations, analysing test results, and diagnosing illnesses.
Physician associates support doctors to diagnose and manage patients in hospitals and GP surgeries.
Care Home Physician Associates have scheduled weekly ward rounds with each care home. During the ward round the care home can expect a high level of personalised care for their residents. Each review is personalised to the patient, taking into consideration their own story, their beliefs, and their choices. We hard work to ensure that Enhanced Health in Care Homes is accessible to all and believe that communication and collaboration is key to ensuring that patients feel connected and listened to.

The Physician Associate is a core member of the monthly multidisciplinary team meeting and if necessary, will present any cases on the care homes behalf.
PCN Pharmacy Team
As a Care Home Service, we work in direct collaboration with our in house pharmacy team on a daily basis. We ensure that structured medication reviews are referred to our highly skilled pharmacists in line with the EHCH and IIF framework and in turn they advise when birth month review have been completed in line with their protocols.
Care homes often request medication reviews on the routine ward round list, these residents are then referred directly to the pharmacy team, bypassing the need for the PA to review the patient unnecessarily.
